All Applicants must have the following:
 

• Valid Florida License

• CNA Certification or HHA certificate showing at least 75 hours of instruction.

• Current CPR

• Level 2 background screening

• Two Professional References

• Auto Insurance Card

• Proof of residency or Citizenship

• Professional Liability Insurance

• Physical Exam: completed within the LAST 6 Months, stating that the applicant is free of all communicable diseases

•  Social Security Card

Requirements:

Complete CEU's - Domestic violence, OSHA, HIV/Aids, CPR, Alzheimer's Certification and a Health statement showing "free from communicable disease" and negative TB test prior to application for employment.
